I started off quite terrible. I couldn't keep up with the beat. But despite that i kept at it. Within less than an hour i was slaying and headbanging to a legendary sound track with ease. I recommend using the revolvers and replaying the second level to get a good feel for the game.

Let the Metal flowwww
This game is awesome! For the fellow metal heads out there it has a solid lineup of vocal artists, and a very welcome fusion between FPS and rhythm gameplay. STOP staring at the rhythm arrows on the screen (you should be looking at the enemies to shoot or evade) and just shoot to the rhythm of what you hear. It takes a bit of getting used to, but that's also what makes it special. There are few Metal games out there, and this one is unique.

Tips in review, awesome game
I've completed the 2nd hell. To this point it's been excellent. I've had zero problems with syncing to the rhythm. Before you even play it has you set your audio and visual timing. It's registering any possible latency and your accuracy.
When action is frantic I've gotten off rhythm. The game is great at notifying you with visible and audio notifications. At that point stop attacking for a beat and reset, no problem. When the vocals kick in at 16x multiplier it is impossible for it to be seamless, but is really close. Again, pause for 1 beat then reset and resume.
As you play you quickly unlock the "range weapon" which is much easier to keep rhythm with. The sword does a finisher after a few hits that takes 1/2 a beat longer. So, you need to pause before swinging again. Keep attacking even when there are no enemies to keep the combo up. Getting hit lowers the combo level and kills are the only way to increase the level.
Finally an original fun game.
